Background

Using Moti Physio For Objective Assessment

The integration of advanced biomechanical imaging with manual therapy and corrective self-care has demonstrably transformed outcomes in physical rehabilitation, neuromuscular re-education, and structural bodywork.

Prior to objective posturographic assessment, our practitioners relied on static visual observation, manual palpation, and subjective functional testing — methods consistently limited by inter-examiner variability.

What We Found in Practice

Clinical findings show that objective assessment has shifted advanced bodywork from interpretive observation toward more measurable and reproducible decision-making. Thai Sport Bodyworks has been using markerless analysis for chronic pain since 2017, starting with Kinetisense before migrating to Phy.Health in 2022.

In Q1 2026 we advanced to Moti Physio, a next generation solution that provides full body musculoskeletal analysis using RGB machine vision that compares favorably with EOS Full Body XRay. It also analyzes functional movement to locate compensation and neurological balance testing. It's proprietary AI analysis measures the activity of 67 muscles involved in posture and locomotion and all major skeletal joints with over 650 million parameters being considered.

Moti Physio conducted over 30,000 assessments of patients against a million dollar full body EOS xray machine, the accuracy goes well beyond anything we need when it comes to functional analysis. It's data outcomes are also directly inline with our Advanced Therapy practice modalities based on Janda and Anatomy Trains Structural Integration.

Historically, most of our chronic pain clients had previously undergone a multi-year year journey in traditional medicine and physical therapy that produced poor results. This is usually due to improper analysis by focusing strictly on the symptoms. In these failed cases, there is likely a complex combination of factors rooted in myofascial and neuromuscular dysfunction located elsewhere that ends up creating pain over the kinetic chain.

This new Moti approach has not only provided a much richer set of data, but enables us to integrate that data effectively within our own patent-pending SmartCare AI/software to provide a wider range of services ranging from injury prevention, better bodywork for maintenance, to resolving chronic pain, and to advanced sport-specific movement and skill optimization.

Why This Integration Changed Outcomes

Markerless 3D posturography provides a level of diagnostic precision previously unavailable in manual therapy settings.

Practitioners using objective digital twin data reported more targeted interventions, reduced treatment trial-and-error, and improved patient communication around structural findings.

FindingsClinical Summary

Core Clinical Findings & Applications

Objective 3D posturographic data can be systematically integrated with established neuromuscular and fascial frameworks to guide precise, mechanism-appropriate manual therapy interventions. In practice, this integration improves clinical targeting, reduces trial-and-error, and strengthens communication around structural findings.

Central Clinical Finding

Objective 3D posturographic data can be systematically integrated with Janda's neuromuscular paradigms and Myers' Anatomy Trains to guide precise, mechanism-appropriate manual therapy interventions — and clinical outcomes improve when it is.

Primary Finding

A clinically actionable framework mapping Moti Physio's red/blue muscle designations onto the pathomechanics of reciprocal inhibition, co-contraction, and long-term structural remodeling was successfully established and validated in practice.

Secondary Finding

Distinct bodywork modalities — deep tissue, NMT, MFR, Thai massage, Spindle-Stim, PNF — were identified as appropriate for each neurological presentation observed on the digital twin, with clear contraindications delineated.

Clinical Contribution

A comprehensive decision matrix correlating postural deviations, fascial meridian involvement, and biologically-grounded tissue remodeling timelines governed by Davis's Law and Wolff's Law was developed and applied across clinical cases.

Interpretive Summary

These findings support a clinically grounded approach in which postural assessment, neuromuscular patterning, and fascial considerations are interpreted together rather than in isolation. The result is a more coherent framework for selecting interventions and anticipating tissue adaptation over time.

Clinical Basis

Observed Clinical Foundations: Janda, Myers & Sherrington

These pillars, applied in clinical practice, consistently transformed subjective palpation into precise, mechanistically grounded multimodal treatment — with measurable improvements in patient outcomes.

Core Clinical Pillars

01

Sherrington's Law

Confirmed neurological basis of reciprocal innervation in observed cases

02

Janda's Syndromes

Upper and lower crossed patterns consistently identified across patients

03

Myers' Anatomy Trains

Myofascial meridians confirmed as structural transmission pathways

04

Moti Physio 3D Twin

Digital twin provided objective diagnostic confirmation in all cases

Key Theoretical Distinctions

  • Goodheart Model: inhibition is primary — treating the weak muscle first produced consistent neurological resets
  • Janda Model: hypertonicity is primary — treating the tight muscle first, as operationalized by Moti Physio, yielded superior structural outcomes
  • Postural muscles tend toward tightness; phasic muscles tend toward inhibition and atrophy

The Binary Visual Map: Confirmed in Practice

Red = hypertonic/overactive; Blue = hypotonic/inhibited. This color scheme directly maps Sherrington's Reciprocal Inhibition onto the clinical interface, eliminating diagnostic guesswork.

Clinical Presentations

Three Examples of Distinct Presentations Observed Clinically

These observations were organized into three recurring clinical patterns, plus a broader fascial architecture finding that explained why local treatment alone often produced only short-lived change.

Finding 1: Reciprocal Inhibition (Red vs. Blue)

Consistently observed: a hypertonic agonist red (tight) muscle chronically suppresses its antagonist blue (weak) through altered reciprocal inhibition. Releasing the red muscle first with deep tissue, NMT, MFR, or Thai massage, followed by Spindle-Stim and PNF CRAC activation of the blue, produced reliable neuromuscular resets.

Finding 2: Co-Contraction & Guarding (Red vs. Red)

Bilateral hypertonicity was identified as CNS-driven protective splinting that overrode reciprocal pathways. Aggressive deep tissue consistently worsened outcomes in this presentation. Strain-Counterstrain, vibration therapy (80–120 Hz TVR), and gentle joint mobilization produced the most favorable responses.

Finding 3: Structural Remodeling & Timelines

Neurological tone resets were observed within a single session; true structural adaptation required weeks to years. Sarcomerogenesis took 3–6 weeks; fascial collagen remodeling spanned months; and osseous Wolff's Law adaptation required 3–6 months to years — findings that recalibrated both clinician and patient expectations.

The Myofascial Architecture: Anatomy Trains Confirmed

Myers' 12 myofascial meridians — SBL, SFL, Lateral Line, Spiral Line, Deep Front Line, Arm Lines, Functional Lines — are confirmed as global strain transmitters. Cases treating only local chronically tight (red) muscles without addressing the meridian tethering of connective tissue consistently produced transient relief on

We offer two therapy session types designed to remodel and improve glide of fascia tissue: For minor issues is the Fascia Realign session. For more serious issues such as chronic pain we use Strcutural Bodywork.

Clinical Decision MatrixApplied Findings

Clinical Decision Matrix: Findings Applied

This section organizes the observed clinical patterns into three of the most common recurring presentations and links them to the tissue adaptation timeline observed across cases. The emphasis is on how clinical findings, fascial behavior, and expected remodeling rates aligned in practice.

Common Clinical presentations

Upper Crossed Syndrome

Red observed: Pectorals, SCM, Suboccipitals, Upper Trap. Blue observed: Deep cervical flexors, Rhomboids, Lower Trap. Fascial finding: SFL shortened, SBL locked long.

Lower Crossed Syndrome

Red observed: Iliopsoas, Rectus Femoris, Erectors. Blue observed: Gluteals, TVA. Fascial finding: DFL adhered at psoas/diaphragm; SBL locked short at lumbar.

Scoliosis Compensation

Paraspinals consistently hypertonic on concave side; locked long on convex side. Spiral Line and Lateral Line severely asymmetric in all cases. MFR + PNF + Sensorimotor Integration produced best outcomes.

Tissue remodeling timelines

Observed adaptation rates varied by tissue type. Neurological tone changed immediately, while muscular, fascial, and osseous adaptation required progressively longer timeframes.

Key FindingsClinical Synthesis

Synthesis of Clinical Findings

The findings below summarize the most consistent patterns observed across cases. The red/blue digital twin provided a clearer clinical decision framework by separating neurological spasm from structural shortening, clarifying fascial continuity, identifying contraindications, and aligning treatment expectations with realistic tissue adaptation timelines.

Neurological Precision

The red/blue digital twin eliminated diagnostic guesswork in practice — distinguishing neurological spasm from structural shortening consistently determined whether aggressive tissue deformation or neurological down-regulation was the appropriate intervention.

Fascial Continuity Insight

Cases treating only the locally symptomatic red muscle while ignoring its Anatomy Trains meridian consistently produced transient results. Lasting structural change was achieved only when the entire restricted fascial line was addressed.

Contraindication Clarity

Co-contraction (red/red) was confirmed as a CNS stabilization strategy. Deep ischemic compression or forced stretching in this presentation reliably triggered increased guarding — the digital twin's objective data prevented this critical clinical error across all observed cases.

Biological Realism

Davis's Law and Wolff's Law were confirmed as governing soft tissue and osseous adaptation over weeks to years. Aligning clinician and patient expectations to these biological timelines — rather than session-by-session neurological tone changes — was a consistent finding across long-term cases.

Interpretive Summary

Across the observed cases, the most important outcome was not simply improved symptom recognition, but a more reliable match between presentation and intervention. Neurological tone changes required different handling than structural adaptation, fascial restrictions needed line-based treatment rather than local-only care, and co-contraction patterns required protection rather than aggressive release. Together, these findings support a more precise and biologically grounded clinical approach.

Conclusion

Conclusions & Next Steps in Clinical Practice

Markerless 3D posturography demonstrated value as a standard pre-treatment assessment across structural bodywork disciplines — broader adoption is warranted based on observed outcomes.

Testing Requirements for Stated Accuracy

To mirror the data environment of the SCIE whitepapers, the testing environment requires:

Fitted Clothing: Loose apparel obfuscates the infrared dots, degrading structural estimation accuracy.

Controlled Lighting: Testing must occur away from direct sunlight, as external UV/infrared rays interfere with the device's depth sensor.
